INSTRUCTIONS FOR COMPLETING THE APPLICATION
Thank you for requesting a scholarship application. Please read the following instructions/criteria before completing.
1. Follow the instructions at the top of the Application, and complete the Application. Please do not send us a picture of yourself, your school transcript, medical records, or your grade records.
2. The Scholarship Selection Committee appreciates more rather than less information about you. When answering the questions, please be as detailed and complete as possible.
3. Give the enclosed link of the Recommendation Form to the person you have chosen to recommend you for the Scholarship. We suggest you ask a teacher, counselor, employer, or coach to write on your behalf. Ask them to submit the Recommendation Form directly to the Hydrocephalus Association.
4. About March 15, check to make sure that the person you have asked for a recommendation remembers to complete and submit it to us no later than April 1, 2012.
5. Submit your completed application no later than April 1, 2012.

7. The recipients of the scholarships will be announced by mail in May 2012.
Remember: Your completed Application Form and Recommendation Form
must be submitted by April 1, 2012. There will be no exceptions.
CRITERIA
1. Applicants of the scholarships must have hydrocephalus.
2. Applicants must be 17 or older.
3. Scholarship funds must be used for an educational purpose, including, but not limited to, a four-year or junior college, a high school post-graduate year to prepare for college, technical or trade school, an accredited employment-training program, or a post-graduate program.
4. The funds may be used for tuition, books, housing, or an expense directly related to the educational experience.
5. Completed Scholarship Applications and Recommendation Forms must be submitted no later than April 1, 2012.
6. The scholarships recipients will be announced in May 2012 and awarded in June 2012.
7. The Scholarship Committee of the Hydrocephalus Association has the sole authority of awarding the scholarships.
8. The amount of each scholarship is $1,000.
9. No financial qualifications will be used in determining the scholarship recipients.
10. The scholarships are non-renewable. Applicants may reapply the following year if he or she is not awarded a scholarship. Previous recipients are not eligible to re-apply.

If you want to work on it over several sessions, please answer the questions in a word processor and then copy and paste them into this form. When you are complete, hit the submit button at the end.
Once you hit the submit button a window with two options will appear:

- Choose Internet Email if you currently use an Internet email services such as Yahoo, Hotmail, Gmail…etc. You will need to save your form to your computer then send it as an attachment to scholarship@hydroassoc.org
